What Medicare Does Not Cover: Understanding the Gaps in Coverage

Medicare covers a great deal, and for many people it is the most dependable health coverage they have ever had. But it was never designed to cover everything. Knowing where the gaps are, before you need care, is what lets you plan for them calmly instead of being surprised by a bill.

Routine dental care

Original Medicare generally does not pay for routine dental care, cleanings, fillings, extractions, or dentures. There are narrow exceptions when dental work is medically necessary for another covered treatment, but everyday dental care is on you unless you have other coverage. Many people handle this gap with a standalone dental plan or, in some cases, dental benefits included in a Medicare Advantage plan.

Routine vision and eyeglasses

Medicare does not cover routine eye exams for glasses or contacts, and it does not pay for eyeglasses in most situations. It does cover medically necessary eye care, such as cataract surgery and treatment for eye diseases. A standalone vision plan is the common way people cover exams and eyewear.

Hearing aids

Routine hearing exams and hearing aids are not covered by Original Medicare. If hearing loss is affecting your daily life, ask us about plans and options that can help with these costs, because they add up quickly when paid entirely out of pocket.

Long term care

This is the gap that surprises families most. Medicare covers skilled nursing and rehabilitation care for a limited time after a qualifying hospital stay. It does not cover long term custodial care, the day to day help with bathing, dressing, and eating that many people eventually need, whether at home or in a facility. Planning for long term care is a separate conversation, and it is worth having early.

Care outside the United States

With limited exceptions, Medicare does not cover health care you receive outside the United States. If you travel internationally, ask about travel medical coverage before you go, not after you are back.

Everyday items and services

Medicare also does not cover cosmetic procedures, most alternative therapies, personal comfort items, or care that is not considered medically necessary. And even for covered care, Original Medicare has deductibles and coinsurance with no annual out of pocket maximum, which is its own kind of gap.

So how do people fill the gaps?

There is no single answer, which is exactly why a review helps. Some people pair Original Medicare with a Medicare Supplement plan and a Part D drug plan, then add standalone dental and vision coverage. Others choose a Medicare Advantage plan that bundles drug coverage and includes some dental, vision, or hearing benefits. Supplemental policies can help with hospital and recovery costs. The right combination depends on your health, your doctors, and your budget.
